Terms and Conditions for lay-by Payment Option

Please read the following Terms and Conditions carefully before proceeding with this payment option. By opting into the lay-by plan, you agree to the terms outlined below.
 

1. Overview of lay-by Payment Option

The Interest-Free lay-by Payment Option allows you to pay for your surgery in advance, with flexible instalments. The full balance must be paid no later than 4 weeks before your scheduled surgery date.

2. Deposit and Payment Plan

• A minimum deposit of $500 is required to start the lay-by payment plan.

• The lay-by option is available for up to 24 months, providing ample time to complete payments.

• You have the flexibility to choose your payment frequency and instalment amounts, provided the total amount is paid 4 weeks before surgery.

• The $500 deposit is non-refundable unless the surgery is cancelled by Cosmex or for medically necessary reasons.

3. No Fees or Approvals

• There are no additional fees or approvals required to use this payment plan. It’s completely interest-free, and no credit checks are needed.

4. Payment Methods

• Payments can be made via bank transfer, credit/debit card, or other approved methods.

• You are responsible for managing and ensuring payments are made on time to avoid any delays to your surgery.

5. Final Payment Deadline

• The total cost of the procedure must be paid in full 4 weeks before your surgery date. Failure to do so may result in the rescheduling or cancellation of your procedure.

6. Changes and Cancellations

• If you need to reschedule or cancel your surgery, please notify us as early as possible. Refunds and fees may apply based on the timing and circumstances of the cancellation.

• Any amount paid excluding the $500 deposit is refundable in accordance with Cosmex’s cancellation policy. 

• The $500 deposit is non-refundable unless the surgery is cancelled by Cosmex or for medically necessary reasons.

7. Your Responsibilities

• By choosing the lay-by payment option, you acknowledge that you are responsible for managing your payments and completing the total payment by the deadline.

• This payment plan is non-transferable and applies solely to the agreed-upon surgery.

• You acknowledge Cosmex and/or Cosmex  is not a payment provider. The payment options offered are intended to provide flexibility. It is your responsibility to ensure you understand and can meet the obligations of your chosen payment plan. We recommend consulting with a financial advisor if you have any concerns. By choosing a payment option, you acknowledge that you have considered its suitability for your financial situation.

8. Refunds

• If your surgery cannot proceed as planned, refunds will be issued in accordance with our standard refund policy, which may include administrative fees.

9. Agreement

By submitting this form, you acknowledge that you have read, understood, and agree to these Terms and Conditions for the lay-by Payment Option. You agree to make timely payments and understand the consequences of failing to meet the payment deadlines.

10. Changes to Terms

Cosmex’s clinic reserves the right to update or modify these Terms and Conditions at any time. Clients will be notified of any significant changes, and continued use of the layby payment option will constitute acceptance of the new terms.

11. Surgery Specifics

Please note that opting for the Layby Payment Option does not guarantee approval for surgery at the time of the procedure. All clients will still undergo an assessment to determine medical suitability for the procedure. Costs may vary at this time.

12. Surgeon Specific Costs

Please note that the total amount applicable for the lay-by plan may vary depending on the surgeon performing the procedure. Be sure to confirm the total cost with Cosmex prior to commencing the lay-by plan.
